How to request consent from guardians for participation in Every Child and Family is Known

Collecting digital consent from families to participate in the Every Child and Family is Known program.

Hello, Caring Adults and Program Points!

The Every Child and Family is Known program is transitioning to collecting program participation consent from families via DocuSign e-signature. This article will show you how to request and manage family consent in the School Portal.

Overview

The Portal is integrated with DocuSign, which allows for secure and seamless digital consent management.

When a consent request is initiated, the primary guardian receives an email with a DocuSign link, enabling them to review and sign the consent document electronically. Once a family's primary guardian has signed consent, they will be able to access the Family Portal, and their consent status will be updated in the Portal.

Consent statuses

Consent status

Description

Not Requested

No consent request has been made for the given family

Pending

An email requesting consent with a link to DocuSign has been sent to the primary guardian, but is awaiting action from the primary guardian

Yes

Consent has been signed and the family is authorized to participate in the Every Child and Family is Known program

No

The guardian has declined for their family to participate in the program

Revoked

Consent that was previously granted has been revoked, and the family may no longer participate in the program

Needs update

Even though consent was previously signed, a new consent document must be signed.

Note: all currently enrolled students must be reflected on the consent form. Consent needs update when a child in the family is newly enrolled in one of the participating schools.

My Families

To view your assigned families, click ECFIK in the lefthand navigation.

On the My Families page, you can view a list of your assigned students, alongside their primary guardian information, program consent status, and links to the student's Family Profile and Student Profile.

Family Profile

To view a student's Family Profile, click the three dot menu next to the student's name and click View Family Profile.

Consent status

View, request, and update a family's consent status in the Consent status section on the lower right of the Family Profile.

On the Family Profile, you can manage a family's guardians.

Managing guardian information

On the Family Profile, you can manage a family's guardians and their contact information.

To be able to request digital consent in the School Portal, the family's primary guardian must have a valid email address added.

Click Add another guardian or Edit information to add guardian emails and phone numbers. Note: guardians must be reflected in ATS as an emergency contact.

Updating consent

Click the three dot menu next to the consent status in the Family Profile to view consent options.

Request consent

Click Request consent to trigger an email to the family's primary guardian with a link to the DocuSign consent form.

When the primary guardian completes the DocuSign, the family's consent status will update to Yes.

Decline consent

Click Decline consent to decline consent on behalf of a family who has informed you that they do not wish to participate in the Every Child and Family is Known program. You can always request consent again should they change their minds.

When consent is declined, the family's consent status will update to No.

Revoke consent

Click Revoke consent to revoke consent on behalf of a family who has informed you that they no longer wish to participate in the Every Child and Family is Known program. You can always request consent again should they change their minds.

Note: the Revoke consent option is only available if a consent document has already been completed for the family.

When consent is revoked, the family's consent status will update to Revoked.
